Program Overview – Seniors Community Programs, Community Care
As a member of the Seniors Community Programs team, the incumbent is responsible to the Manager for day-to-day coordination of outreach and engagement initiatives associated with the Seniors Active Living Centre and Outreach (SALCO) portfolio.
This role is designed to build community capacity within the WoodGreen seniors’ community by developing the leadership, networking and organizing skills of seniors through supportive training, coaching and facilitation. The role will support the development of sustainable community-based, seniors-focused programming that is designed to promote wellness with focus on community engagement, seniors’ safety and outreach to vulnerable seniors. Programs/projects within the outreach team include friendly visiting/ medical escorts, ESIG grant program, food security initiatives and ongoing outreach and partnerships within the Seniors Community Programs portfolio.
What You Will Do
- Provide direct supervision to a small team of coordinators in the outreach team portfolio.
- Lead annual planning, monitoring, and evaluation of the outreach projects and ensure they meet organizational objectives and community needs.
- Conduct regular assessments of practices, identifying areas for quality improvement and recommending actionable solutions to ensure sustainability of the programs.
- Use evaluation methods and reporting tools to gather and maintain statistical information on activities and programs for WoodGreen and associated funders.
- Provide administrative leadership for the ESIG program, including oversight of the application process and ongoing collaboration with established community organizations.
- Support the Manager in identifying and pursuing grant opportunities to strengthen programs and address community needs.
- Support new partnerships and projects within the SCP portfolio
- Develop and maintain contacts with seniors’ groups, community agencies, and providers and attend external meetings as needed.
- Develop outreach strategies including outreach materials and educational presentations around
- seniors’ safety and other emerging age-related issues.
- Develop and implement a process for acquiring and responding to client input, including project consultation with outreach coordinators.
- Manage and resolve all client complaints according to the Unit and agency-wide policies and procedures.
- Ensure discretionary program purchases are consistent with delegated budgetary allocations and in accordance with WoodGreen and funder restrictions.
- Ensure the effective utilization of volunteers and students.
- Hold regular supervisory meetings with outreach team members, identify training and development needs, opportunities, and recommend disciplinary action when necessary.
- Ensure the planning and implementation of a staff development program.
- Participate in regular supervisory sessions with the Manager.
- Ensure the recruitment and hiring of qualified staff under established recruiting policies and procedures and the Collective Agreement.
- Represent the unit on internal staff committees as directed by the Manager.
- Ensure the anti-racism objectives and community development principles of the organization are achieved within the program.
- Responsible for upgrading training and education as directed by WoodGreen Community Service to fulfill the responsibilities of the job.
- Maintain familiarity with WoodGreen’s emergency procedures and staff telephone numbers.
What You Bring to the Team
- Bachelor’s degree in health or social science discipline from a recognized University and/or an approved equivalent of education and related experience
- Three to five years’ related experience includes significant direct experience in working with older adults within a community setting.
- Experience in community development, peer and volunteer-led programming, training facilitation or co-design.
What Will Set You Apart
- Good understanding and knowledge of seniors’ issues related to social, recreational and educational needs.
- Good understanding of the social determinants of health in relation to vulnerable and marginalized seniors.
- Knowledge and demonstrated skills in planning, implementing and evaluating seniors programming
- Able to work independently with minimal supervision.
- Knowledge of community resources for older adults.
- Second language is an asset.
- Capable of fulfilling the physical and mental demands of the job as per the Position Demands Analysis.
