Program Overview: Community Care - Home & Cluster Care
It is the mission of the Community Care and Wellness for Seniors Unit to provide a broad spectrum of services to enable seniors and people with disabilities to continue to live independently. The Supportive Housing and Homemaking (SH & HM) programs provide support services to this population. Staff may also be required to serve persons with a history of mental illness, addictions, substance use and who have lived in shelters/hostels (the “hard-to-house” population).

Homemaking services are funded in several ways: Metro
Community Services and partial funding from the Ministry of Health for Home Help (clients pay for this service).
Supportive Housing is funded by the Ministry of Health thro’ the Ontario Health at Home.


What You Will Do
  • Complies with funder policies and procedures (e.g. HMNS and Ontario Health atHome).
  • Follows the care plan of the client as instructed by the supervisor.
  • Documents accurately in the appropriate records e.g. grocery form in a timely and professional manner.
  • Ensures she/he has a sufficient supply of personal protective equipment e.g., gloves, bed bug suits, especially if HHW engages in bedbug preparation work in client’s home which is infested with bedbugs.
  • Ensures there are sufficient cleaning tools, materials and cleaning supplies to perform house cleaning, light housekeeping and laundry duties. 
  • Follow safety procedures as per policy and ensure personal safety if travel involved going to laundry rooms in the apartment building of where client lives.
  • Completes the tasks and assignments outlined in the care plans of the clients within the hours scheduled for work with client, call supervisor immediately if additional hours or time needed to finish the care plan assigned. Additional hours of work need to be authorized or else the HHW would not be paid.
What You Bring to the Team
  • Minimum 6 – 12 months direct experience working with frail elderly, or persons with disabilities.
  • Recognizes that she/he is assigned to work based on program needs, client preferences and HHW skill set as required.
What Will Set You Apart
  • Recognizes that services are client-focused and delivered in a professional and culturally sensitive manner.
  • Complies with all agency, unit and program policies and procedures as required and as communicated e.g., during orientation, at training, memos, meetings/minutes, voice mail hot line, reference manuals. This also applies to policies and procedures at other sites where the HSW must work closely with other partner agency staff e.g., PSWs of other service providers.
  • Provides service to clients relevant to the program and as authorized by the Supervisor and within the scope of practice as permitted by his/her training and follows all WoodGreen/Unit and Program policies and procedures.
  • Complies with all health and safety policies and procedures, such as:
    • promoting a healthy and safe workplace environment by identifying and responding to actual or potential health and safety hazards and emergency situations e.g., managing a client emergency (e.g., illness, fire in home), an HHW emergency (e.g., sudden injury at work, TTC strike) or an emergency that affects program operations (e.g., power failure in cluster).
    • consistently applying Routine Practices (Infection Prevention and Control (IPAC)).
    • using personal protective equipment (PPE) e.g., N95 mask as required.
  • Able to provide service based on physical demands of the job which require physical strength, stamina and ability to undertake frequent light to moderate lifting (10-25 kgs), and frequent walking, bending, stretching and pulling and able to operate equipment (e.g., vacuum cleaner, mechanical lift). 
  • Reports any injury received at work to the Supervisor/On-call within 3 hours on the day of injury and/or any perceived injury that may occur because of work and if required, participates in an “early and safe return to work” program.
  • Obtains schedule by Friday prior to the week of service, responds to all office voice mail message whether HW can do the work offered.
  • Uses the Alayacare Mobile Application on their cell phone to clock in and out of their scheduled visits/shifts.
  • Presents a professional image to clients and the community (dress code, identification), and conduct him/herself in a professional manner when engaged in all work-related duties, including travelling to or from a client’s home or carrying out business e.g., interactions with other health care professionals, medical escort (if applicable).
  • Uses good communication and interpersonal skills to work effectively with program staff (e.g., Scheduler, Supervisor), co-workers, other service providers (e.g., Fife House, Ontario Health atHome, nursing, social work, funder etc.) and clients as required. This includes promoting a positive workplace environment and team building with co-workers when working in sites where teamwork is vital e.g. Broadview Manor. This also applies to resolving or getting assistance with any conflicts when they arise.
  • Communicates with office (scheduler, supervisor/on-call) as per policy – e.g.
    • changes to schedule (e.g., not seen not found visit, lateness > 10 minutes).
    • changes in a client’s physical, emotional and social status.
    • actual or potential client/HSW health and safety hazards in the work setting.
    • when potential or actual conflict arises with client or client’s family/caregiver.
    • incidents or complaints related to service delivery.
    • Changes in HSW personal information (e.g., work availability, address, and tel. number).
  • Participates in regular supervisory sessions with the Supervisor.
  • Attends training and meetings as directed by Supervisor to fulfill job responsibilities.
  • Recognizes that WoodGreen has a zero-tolerance policy regarding harassment and racism and complies with this corporate policy and conflict of interest and confidentiality policies; abides by corporate code of conduct.
  • Seeks clarification from Supervisor for any work-related incidents, issues, policies, concerns.
  • Other duties as assigned by the Supervisor.
